Health Questionnaires in the Patient Portal
Our platform has been enhanced with new features to help your healthcare provider manage your health more effectively. A health questionnaire is a structured set of questions designed to gather relevant health information from patients. These questionnaires can cover a range of topics, including current symptoms, lifestyle factors, and mental health status.
By filling out questionnaires beforehand, you can save time during your appointment and allow healthcare providers to focus on discussing the results and planning the next steps.
Here’s how to use the Patient Questionnaires feature to complete and review health questionnaires online.
Accessing Questionnaires
After logging into your Patient Portal, locate and click the Patient
Questionnaires tab to access your questionnaires.In the Patient Questionnaires
section, you will find two tabs: Available and
Completed.
- Available Questionnaires:
Lists all the questionnaires that you need to complete. - Completed Questionnaires:
Shows all the questionnaires that you have already filled out and
submitted.
Completing Questionnaires
In the Available tab, view the list of questionnaires awaiting completion.
1. Click Complete next to the questionnaire you wish to fill out. This action will open the
questionnaire for you to complete.
2. Carefully fill out all sections of the questionnaire.
3. After completing the questionnaire, click the Submit button at the bottom of the form.
4. Once submitted, you will receive a confirmation message indicating that your responses have been successfully
submitted to your healthcare provider.
You will have the option to view the completed questionnaire by clicking on the confirmation message or by navigating to the Completed tab and clicking the eye icon next to the questionnaire.
