Can I edit a group note after it has been signed?

Answer: No, once a group note is signed, it becomes permanent and cannot be edited. This practice ensures the integrity and legal compliance of the documentation.


If I make changes to a group note after exporting it to individual patient charts, will those changes update in the individual notes?

Answer: No, once a group note is exported to individual patient charts, any subsequent changes made to the group note will not be reflected in the individual notes. To ensure consistency, make all necessary edits to the group note before exporting it. After exporting, any additional changes must be made individually in each patient’s chart.


What happens to the HPI text in an individual note if I apply an HPI template?

Answer: Applying a Global and/or HPI template to an individual note will overwrite any existing text in the HPI section of that note.


Can I select different group note formats or choose the default individual note type upon export?

Answer:  Currently, the system does not support selecting different group note formats or choosing the individual note type upon export. This functionality is planned for future updates. In the meantime, you can switch to a different note format within each patient’s chart after exporting the group note. This allows for customization of individual notes as needed.


What is the difference between Shared Notes and Patient-Specific Notes?

Answer: Shared Notes are for information relevant to all attending patients and will appear in each patient’s individual note. Patient-Specific Notes allow you to document information unique to each patient, which is saved to their individual record.


Will Shared and Individual Notes carry over to the HPI section when switching note formats?

Answer: Yes, Shared and Individual Notes will carry over to the HPI section when switching between different note formats, provided that the new format includes an HPI section. This ensures continuity of information across various documentation styles.


Will Axis & Plan and Current Procedures entries carry over when switching note formats?

Answer:  Yes, entries in the Axis & Plan and Procedures sections will carry over when switching between different note formats, as long as the new format includes these sections. This maintains the integrity of the clinical information throughout various documentation formats.


What happens when I export the Group Note to individual charts?

Answer:  Exporting transfers all Shared Notes, Patient-Specific Notes, Axis & Plan, and Procedure Codes to each patient’s chart. Make sure all information is complete before exporting, as this action can only be performed once and cannot be undone.


How do I create a Group Note for a session?

Answer: To create a group note for your session, simply follow these step:

Step 1: Login to WRS-EHR and go to the Today’s Appointment List.

Step 2: Locate the specific group appointment you want to create a note for.

Step 3: Hover over and click the “Create Group Note” button. This will open the group note interface where you can begin documenting.

Can I track attendance for each patient in a group session?

Answer:  Yes. In the attendance list within the Group Note, simply uncheck the box next to a patient’s name to mark them as absent. The interface will automatically update, striking through the names of patients marked as absent and removing their individual sections.

How can I add Axis & Plan information for the group?

Answer: To add an Axis and Plan information, 

Step 1: In the patient note sidebar, open the Group Axis & Plan section.

Step 2: Enter relevant Axis IV and Axis V information. You can also add free text assessments and select ICD codes.

Step 3: Add any treatment notes or next steps in the Discussion field.

Step 4: Click “Save” to ensure all Axis & Plan entries are saved.

How do I add Procedure Codes to a Group Note?

Answer: To add a Procedure,

Step 1: Navigate to the Group Procedures section.

Step 2: Enter the CPT code directly or select it from your superbill. These codes will be transferred to each patient’s individual note for accurate billing documentation.

Step 3: Click the “Add” button to add the code to the Current Procedures list.

Step 4: Add any additional comments if needed and click “Save” to complete the process.
