E- Prescribing Service Levels

E-prescribing is a safer and more efficient way for healthcare professionals to manage electronic prescriptions. The Surescripts E-Prescribing service helps Prescribing Providers send e-prescription messages from their system to the patient’s pharmacy of choice.

Enrollment

To enroll, prescribers need to fill out the Surescripts Prescriber Identity Attestation Form completely. They should also provide a copy of their Medical License and a valid Government ID. In some states, a copy of the Drug Enforcement Administration (DEA) license may also be necessary for enrollment.

Prescribers who have been enrolled with Surescripts previously need to provide demographic details that align with Surescripts data. This includes Surescripts Prescriber Identifier (SPI), National Provider Identifier (NPI), Provider First Name, Provider Last Name, Address, Primary Phone (preferred), Secondary Phone (accepted), and Fax Number.

When you are set up with the WRS system, the features of New Prescription (New Rx) and Service Levels such as Refills, RxCancel, RxChange, and RxFill will be activated automatically. Nevertheless, adjustments can be made to Service Levels upon request. If you decide to deactivate certain service levels, you can either create a support ticket or contact your Senior Solutions Engineer directly.

Accessing Electronic Routing

In the Electronic Routing Setup, the administrators can view the Prescribe-On-Behalf-Of and Rx Service Levels. Prescribers with administrative access can verify Rx levels by navigating to:

  1. Administration Tab > Resource Management > Resource Setup.
  2. Select the Prescriber’s Name.
  3. Click on the Electronic Routing Tab.

Rx Service Levels Information:

New Prescription

  • The New Prescriptions capability allows prescribing users to send new prescriptions created within the system. New prescriptions will then be routed from the WRS system to the pharmacy of choice.

    NOTES

    Prescribers desiring to send an additional NewRx prescription(s) will send New Rxs with an effective date set based on the value entered in the duration field.

Refills

  • The Refill Request originated from the pharmacy. This message is to request approval for additional refills of a prescription.
  • A Refill Request response is sent by the prescriber to the pharmacy in response to a request to refill a prescription. The response indicates whether the Refill Request has been Approved, ApprovedWithChanges, Denied, or Replaced.

    NOTES

    Prescribers should review all requests. Prescribers should not delete a request message from a processing queue without a response being generated to the requester. Prescribers should respond to Refill Requests within 48 hours.

  • In Refill Request the total number of dispensing is only the refill quantity indicated when the medication was prescribed.

Rx Cancel

  • The CancelRx is used to notify the pharmacy that a previously sent prescription should be canceled and not filled. The message originated from the WRS system as a CancelRx. The CancelRx Response is sent from the pharmacy to the prescribing system in response to a CancelRx.

Rx Change

  • An RxChange Request message is originated by the pharmacy to request a change to a new or fillable prescription.
  • Response types for RxChange are Approved, ApprovedWithChanges, Denied, and Validated.
  • An RxChangeRequest may be sent with the following Message Request Code(s):

    “G” (Generic Substitution), may be used to request a prescriber allow the dispensing of a generic medication when substitution is not allowed by the prescriber or regulations.

    “P” (Prior Authorization Required), may be used to request a prescriber review the drug requested and obtain a prior authorization from the payer for the existing prescription.

    “T” (Therapeutic Interchange/Substitution), may be used to request a prescriber authorize a therapy change to the original prescription. This includes the workflow where a pharmacy requests a 30-day to 90-day quantity or a formulary change.

    “D” (DUE – Drug Use Evaluation), may be used to request a prescriber authorize a change to the patient’s current medication therapy, such as a dosing regimen change or an alternative medication that will have fewer, or no, adverse effects than the original prescription.

    “S” (Script Clarification), may be used to request a prescriber clarify the original prescription.

    “OS” (Out of Stock), may be used to request a prescriber authorize a change to the original prescription due to the pharmacy being out of stock of the requested medication.

    “U” (Prescriber Authorization), may be used to request prescriber authorization information, such as confirming their DEA number which is the identifier number assigned by the DEA to allow the provider to prescribe controlled substances or their enrollment with the prescription benefit plan.

    Denied and Validated are the only acceptable RxChange Response types for this workflow.

RxFill

  • RxFill is used to provide prescribers with information regarding dispensing activity for a prescription.
  • There are three common use cases to indicate the need or desire to receive RxFill:
    1. To inform prescribers based on a request for notification of the dispensing-related activity for adherence monitoring typically used in a retail setting.
    2. Used for reconciliation purposes to synchronize the care setting or service application with the pharmacy dispensing information typically in the Long-Term and Post Acute Care (LTPAC) setting.
    3. The message can be sent by the pharmacy as part of required regulatory notifications that may be sent regardless of the RxFillIndicator options sent within a message.
