Tax ID, NPI & CLIA Numbers

GO TO

ADMINISTRATION → BILLING SETUP → TAX ID/CLIA SETUP

STEPS

  1. Enter your Practice Tax ID and NPI, making sure to provide a unique CLIA # for each location
    1. If you don’t have an NPI, you can apply for one on the NPPES website
    2. If you need to obtain a CLIA #, go to the CMS website

NOTES

  • These items are used to populate your CMS 1500 claim form
  • Your NPI describes your Group Name
  • Your CLIA is used to bill for lab services (CLIA=Clinical Laboratory Improvement Amendments)
