HPI TEMPLATE CREATION

To Create a History of Present illness (HPI) Template:

GO TO

Administration > EMR setup > HPI Templates.

STEPS

Step 1 : Scroll to the bottom to create a new template.

Step 2 : In the Title box, enter a name for your template.

Step 3 : Select the type of content you would like for your template, utilizing two features:

  • DB fields: Allows you to insert demographic data from the patient chart (color: Yellow). Data will automatically be reflected in the HPI field of the note according to the patient’s demographic information.
    • Current Time
    • Today’s Date
    • Patient’s first name
    • Patient’s last name
    • Patient’s age
    • Patient’s sex
    • Patient’s DOB
  • Choice tags: Enable you to create choice tags, providing choices as answers for the question you added.

Step 4 : Select the type of choice tag. There are two types of tags:

  • Multi select YES: Allows the user to choose more than one answer (color: purple).
  • Multiselect NO: Implies there is only one answer to the question (color: blue).
  • Default Value YES: Allows the user to set a default option.
  • Default Value NO: Implies there is no default option.
Note: Default Values can be used simultaneously with the functionality of both Multi Select YES and Multi Select NO. You can “add” up to 15 choices by clicking “add one more”.

Click Create.

Step 5 : Click the Add button to add your newly created template.

Once you create a template, you can use the same steps to edit an existing template.

NOTES

  • It is best to be in google chrome when creating HPI templates. HPI templates created are user specific. If you would like to share your created HPI templates to another user, use the Template Clone Tool in EMR set
