Completing & Attaching a Questionnaire within an EMR Note

A health questionnaire is a structured set of questions designed to gather relevant health information from patients. These questionnaires can cover a range of topics, including current symptoms, lifestyle factors, and mental health status. By collecting detailed and comprehensive data about a patient’s health, questionnaires ensure that no critical information is overlooked.

To fill out a questionnaire yourself, navigate to the Questionnaires page within an unsigned note.

1. Create a new note or navigate to the applicable unsigned note.

2. Click the Questionnaires button. 

3. In the Patient Questionnaires section, you will find two tabs: Available and Completed.

    • Available Questionnaires: Lists all the questionnaires that you can complete.
    • Completed Questionnaires: Shows all the questionnaires that have already been filled out.

4. To complete and attach a questionnaire to the open note, simply select Complete & Add to Note.

5. Fill out the questionnaire in the new window that appears.

6. Click Submit once you are done. 


7. A confirmation message will show that the questionnaire has been added to the note. 

8. The submitted questionnaire can be viewed in the Completed tab or by selecting the View Note option.

Note: If necessary, you can remove an attached questionnaire from an unsigned note by clicking the Delete button on the View Note or View All Notes page.
